ONU Memorial Tree or Bench Donation Form

The planting of trees or placement of a bench is a long-standing tradition to commemorate loved ones or members from our academic community. Whatever the reason, trees, benches and similar memorials are a valued addition to our campus.

Planning ahead is an important part of donating a tree or bench to the ONU campus. Facilities management needs several weeks advance notice to coordinate the location of a bench or to plant a tree. The location for a bench or tree is determined by The V. P. for University Advancement working with the facilities team. The tree variety that can be planted is determined by the department chair for biological and allied health sciences.

Process for Donating a Memorial Tree or Bench

The following steps are necessary to complete a tree or bench donation:

• Donor should contact the VP for University Advancement at 419-772-2035
• Complete the Memorial Tree or Bench Donation Form below or download and print the pdf form and return it with a check payable to Ohio Northern University. Please mail to: The VP for University Advancement, Ohio Northern University, 525 S. Main Street, Ada, OH 45810
• A location map will be mailed to the donor after the tree is planted or the bench is placed.
